Michael Pittman, MD is a solo practitioner of psychiatry, and he does not have a billing department.  At this point he does not take any insurance assignment, including Medicare or Medicaid.

 

Our financial policies are as follows:

1. Payment in full is due at the time of service.
2. Payment may be made in the form of cash or personal check.  Most major credit cards accepted.
3. If a check is returned Insufficient Funds you will be asked to make future payments to be in cash, at least until credit card acceptance is established.
4. When payment for services is made we will provide a statement documenting the diagnosis and services provided.  This can be submitted to your insurance for out-of-network reimbursement.

Fee Schedule:

1. New evaluation--about an hour................................................................$215
2. Individual Psychotherapy (20-30min), without medication management.......$ 85
   
3. Individual Psychotherapy (20-30min), with medication management............$ 95
4. Individual Psychotherapy (45-50min), without medication management......$125
5. Individual Psychotherapy (45-50min), with medication management...........$150
6. Individual Psychotherapy (75-80min), without medication management......$200
7. Individual Psychotherapy (75-80min, with medication management............$225
8. Telephone visit (scheduled or urgent).........................prorated @ $250 per hour
9. Late cancellations or no shows...............................................................$ 50

Missing or running out of psychiatric medicines can sometimes cause a relapse of the illness being  treated; also, many psychiatric medicines have withdrawal syndromes that can range from unpleasant to dangerous.

 

Our refill policies are as follows:

1. Please keep track of the medicines remaining, and a few days before the last remaining dose, ask you pharmacy to fax a refill request to the office.
2. Some medicines, such as stimulants, require an actual written prescription to  be presented to the pharmacy.  Please call the office for this; the prescription can be mailed or picked up
3. Urgent prescription refills can be obtained by calling the office.
4. Dr. Pittman does not refill medicines prescribed by other doctors.

Missing an appointment is not a good idea, but sometimes it does happen.  An appointment is like a reservation of the doctor's time and missing it is a disservice to other patients.  

Our missing/late cancellation polices are as follows:

Please make changes in appointments at least 24 hours before it is scheduled, not including holidays and weekends, to avoid the late cancellation fee.

If you are a competent adult, of course, you have the right to terminate your care with me at any time.

 

Our termination policies are as follows:

It is my responsibility to treat you in the best way that I can, but, in order to do this, you have to take some responsibility for your care.  If I am prevented from providing the best care that I can by your engaging in an illegal act related to my treatment of you, such as providing others with medicines, especially psychostimulants, that I prescribed to you, or by repeated irresponsible acts I will be forced to consider termination of treatment.

Irresponsible acts can include:  Many missed appointments or late cancellations, failing to schedule and keep follow-up appointments as required for necessary treatment monitoring, failure to follow recommendations for treatment including recommendations for psychotherapy, chemical dependency issues, or medical conditions that require assessment/care for maintenance or good health, or non or frequently late payment of fees for services provided.

If these sort of things occur, they will be discussed with you and attempts to rectify problems will occur before termination would be decided upon.

Termination of care will occur by letter notification.  If I believe it is necessary, a thirty-day prescription of medication will be provided to assist in the change to a new doctor.
